1. You must have your student ID # before starting the online process.

2. go to rankonesport.com

3. Under the "For Parents" tab, click "Fill out Electronic Forms".

4. On the Electronic Forms Lockup" screen, select "OK" from the drop down menu.

5. Scroll down and select Little Axe Public Schools.

6. Click on the "Start Online Forms" button at the bottom of the screen. 

7. You will have the option to create an account, but do not have to. You can create an account, or select "I don't want an account, go to forms". 

8. Fill out student first name, student last name, student ID, and select which school your child is attending.

9. STUDENT EMERGENCY INFORMATION FORM.

-Fill out all of the information on the form

- Use your mouse to sign in the "Student Signature" and "Parent/Guardian Signature" blocks.

10. ATHLETICS PARTICIPATION FORM

- Click on the hyperlinks for "Concussion Acknowledgment" and "Sudden Cardiac Awareness Acknowledgement". After you have read the forms, click on the box net to each of the hyperlinks. This signifies that you have read the information.

-Fill out all the remaining information on the page and, using your mouse, sign in the "Student Signature" and "Parent/Guardian" signature blocks. 

11. DRUG TESTING CONSENT FORM (Middle School Students do not need this form). 

- You may click on the blue highlighted hyperlink "Drug Testing Policy" if you would like to read the entire drug policy. 

- Fill out the information on the form and, using your mouse, sing in the "Student Signature" and "Parent/Guardian Signature" blocks.

12. EMERGENCY AUTHORIZATION

-Fill out all the information.

-Using your mouse, sign in the "parent/Guardian Signature" block.

13. OSSAA Physical Consent Form

-This form will need to be filled out and returned to the athletic office. 

-Under the download and print tab, click on the OSSAA Physical and Parent Consent Form

ALL FORMS MUST BE FILLED OUT BEFORE AN ATHLETE IS ALLOWED TO PARTICIPATE IN A SPORT. 

If you do not have internet access please contact the high school office and we will make arrangements for you to fill out the electronic forms on campus.
